BIRTH AND DEATH CERTIFICATES
BIRTH CERTIFICATES
Pennsylvania charges $10 per Birth Certificate. These are the certificates referred to having the “raised” seal. If you were not born in PA, you can contact us for further information. Please have the following information ready:
The full name of the person, at birth
The Father’s full name
The Mothers MAIDEN name
The person’s birth date
The Hospital name where the person was born
The County name where the person was born
The Borough, City or Township the person was born in
Your current address and phone number
A Valid Government issued ID that verifies both the name and a current address.
If you are a veteran, the spouse of a Veteran, or the minor child (under 18) of a Veteran, you will also need the Veteran's full name, current address, service number, and rank at discharge. The state provides free Birth Certificates to all veterans, their spouses, and their children less than 18 years of age.
The normal processing time for us to obtain your Birth Certificate is four to five business days but can take up to 15 working days. For this service, we charge an office fee of $20.
DEATH CERTIFICATES
Pennsylvania charges $9 per Death Certificate. These are the certificates referred to having the “raised” seal. If the death was not in PA, you can contact us for further information. Please have the following information ready:
The full name of the person, at death
Date of death
The County name where the person died
The Borough, City or Township the person died
The Father’s full name
The Mothers MAIDEN name
The person’s birth date
Social Security Number of deceased
Funeral Director
Your current address and phone number
Fees are waived for individuals who served or are currently serving in the Armed Forces and their dependents. You will need the Veteran's full name, current address, service number, and rank at discharge and the relationship to the Armed Forces Member.
